The sand in this isn't sand as you typically think of it. It is very fine spherical beads, it is not rough in the slightest, it feels more like a lubricant than "sand". This sort of system (with a air permeable sheet over the top) is used as a bed for patients with significant wounds, as it results in extremely well distributed pressure and the flow of air with the air permeable sheet keeps the wound dry.
